New Patients

Please arrive 30 minutes prior to your appointment to complete your registration forms.

Follow-Up Patients

Please arrive 15 minutes prior to your appointment to fill out your forms. Please bring copies of previous medical records and test results for the doctor to review and retain. This includes all relevant CT and MRI pictures (these will be returned to you at the end of your visit).
